AB003. Laparoscopic common bile duct exploration: 1 year experience using pediatric bronchoscope

Background: The aim of this study was to evaluate the safety and feasibility of laparoscopic common bile duct exploration (LCBDE) through choledochotomy with T-tube and transcystic for common bile duct stones (CBDS) using pediatric bronchoscope.

Methods: Between January 2017 and January 2018, a total of 12 patients with CBDS stones undergoing LCBDE at Prof R.D. Kandou Hospital, North Sulawesi, were registered prospectively. All patients were followed up by review of the patient records according to a standardized protocol.

Results: No severe intraoperative complications were registered. One procedure after LCBDE required conversion to open laparotomy due to mass in cystic duct and gallbladder and one procedure were converted to open surgery due to impacted stone. The mean operative time was 311.66 min. The mean postoperative hospital stay was 7.25 days. None of the patients experienced retained stones, biliary peritonitis, biliary fistula, pancreatitis, or cholangitis. None of patients readmitted with obstructive jaundice and clinical sign of stricture. No death within 30 days after surgery was seen.

Conclusions: Laparoscopic cholecystectomy and common bile duct exploration with choledochotomy and transcystic approach is a highly successful procedure for the management of common bile duct stones in an unselected group of patients. The use of Pediatric Bronchoscope with ERCP extraction Basket can be the alternative of choice as a method of common bile duct exploration.
